The starbursts belong to the illustration's original clinical purpose — they are the generic pain or irritation markers used in chiropractic and physical-therapy diagrams. The green and red lines are the annotations added afterwards, and they are what makes this a working file rather than a stock image: someone has drawn a vertical reference and a candidate angle across a standard anatomy plate.
This image is the base layer that several of the other diagrams in this cluster build on, which is why it is worth looking at before them. Once you know which lines came with the illustration and which were drawn on top, the derived graphics are much easier to read.
