The two annotations make one claim between them: an entry or impact point at the circled position, and a direction of travel indicated by the arrow. That direction runs downward and forward across the neck rather than steeply down from above, which is the same argument the diagrams in this cluster make.
The frame itself cannot support the identification. At this level of enlargement from compressed footage, the circled area is a handful of pixels, and hair, shadow, an earpiece, and a microphone cable all read the same.
