The red line is the proposed path. Drawn at that angle it passes across rather than steeply down, which is precisely the point being made: a shot from an elevated rooftop at range should descend, and the annotator is arguing that the described injury pattern implies something much closer to level.
The reliability of the reading depends on how accurately the skeleton has been scaled and positioned over the photograph, and on the subject's head angle at that instant. Both are estimates.
